for Medication
Please complete
the following forms if your child will need medication while on this trip.
Prescription
Medication
Physician's Authorization Form - this
must be signed by a physician and by the parent. If
you currently have one on file with our school nurse, indicate that
with a note on the top of this form but do not complete it. A copy will
be made of the one on file.
Nature's
Classroom Medication Administration Form
All medication,
prescription or non-prescription, must be administered by the Nature's
Classroom medical staff only.
Non- Prescription
Medication
Nature's Classroom Medical Permission Slip
(complete and check yes). There
is a copy of this form attached to the bottom of the registration form.
